Do Yoga and Meditation Reduce Health-Care Utilization? What Studies Say

Countless studies have documented the many health benefits of both regular practices of yoga asanas as well as meditation.

The intriguing question is – does that translate into lower health care utilization?

Indeed, over the past decade, several studies have suggested that mind-body interventions like yoga and meditation not only are highly effective in reducing stress and improving well-being, but can also lead to lower health care utilization. In this article we review some of the more comprehensive research in this area.

Study Examines Relaxation Response Resiliency Program and Stress

Chronic stress undermines our physical and psychological health and is one of the leading causes of medical visits. While most traditional medical interventions target disease symptoms, very few address one of the number one causes of illness: chronic stress. The Relaxation Response Resiliency Program (3RP) is designed to do just that—teach patients how to decrease stress and increase resiliency. In theory, programs such as these should lead to decreased stress, fewer health complaints and considerable reductions in health care service utilization.

Researchers at Massachusetts General Hospital Benson-Henry Institute tested this hypothesis by examining the medical records of patients who had either received 3RP training (n =4,452) or no-treatment controls (n  = 13,149) between January 2006 and July 2014.

Records one year before and after 3RP training were examined for the intervention group and compared to those of a control group of patients who were matched to the 3RP group by age, ethnicity and gender. Key outcomes of interest included the number of billable medical encounters per year (doctor visit, imaging, laboratory, medical procedure, ER visit etc.).

Participants in the 3RP group included any patient who was referred to either individual or group training in 3RP at the MGH Henry-Benson Institute during the study period. The overarching goal of 3RP is to reduce the stress response, elicit relaxation and increase resiliency.  The eight-session intervention integrates relaxation, meditation, mindfulness and cognitive skills training with social support and positive psychology.

When comparing the 3RP and control samples it was discovered that those in the 3RP group were more likely to identify as Asian versus African-American, to identify as Jewish or having no religious affiliation, and to have a higher median income than control participants. Both groups spent approximately the same continuous length of time (more than four years) within the healthcare system.

Healthcare utilization differences between the two groups were significant, with those in the 3RP group averaging a 43% reduction in billable medical and/or diagnostic visits. On the other hand, control group members demonstrated increases in the number of outpatient and specialty care visits and hospital admissions. The authors of the study estimate that reductions in service utilization in the 3RP group translate to a median cost savings ranging from $640 – $25,500 per patient per year. This represents a considerable reduction in medical costs, particularly for those suffering from chronic illness.

Transcendental Meditation – Setting a Gold Standard for Healthcare Cost Reduction

While the Relaxation Response Resiliency Program (3RP) demonstrated impressive results with a 43% reduction in healthcare utilization, Transcendental Meditation (TM) stands out as the most rigorously studied meditation technique for healthcare cost reduction, with over 600 published studies and even more dramatic results1, 2, 3.

 

Landmark Healthcare Utilization Studies

The Blue Cross/Blue Shield Studies represent the most comprehensive research on TM and healthcare costs to date. Two major field studies using Blue Cross/Blue Shield of Iowa insurance statistics examined TM participants compared to approximately 600,000 Blue Cross members and other comparable groups456.

The results were remarkable:

  • 87% lower hospitalization rates for diseases of the heart and blood vessels678

  • 55% lower overall admission rates for all causes compared to other groups47

  • 61% fewer patient days per 1,000 enrollees4

  • 53% fewer outpatient visits4

  • 68% and 74% reductions respectively for TM practitioners over age 404

Even more impressive, a focused study on participants over 45 who practiced TM showed 88% fewer total hospital days over an 11-year period and approximately 60% lower total medical expenditures compared to controls 4.

 

Quebec Longitudinal Cost Studies

The most significant longitudinal research comes from Quebec, Canada, examining government health insurance data over periods spanning 6 to 14 years. A study of 163 TM practitioners over age 65 compared to matched controls revealed a 69.84% five-year cumulative reduction in physician payments relative to controls4. This finding is particularly relevant for the over-50 population, demonstrating TM’s exceptional value for older adults.

 

American Heart Association Recognition

TM is the only meditation technique specifically recognized by the American Heart Association for clinical consideration in treating high blood pressure 9, 10, 11, 12.

 

After systematically reviewing the scientific literature, the AHA concluded that TM produces clinically meaningful reductions in blood pressure of approximately 4.7 mmHg systolic and 3.2 mmHg diastolic911, changes that translate to substantial reductions in cardiovascular events and associated healthcare costs.

 

A landmark cardiovascular study of 201 African American men and women with coronary heart disease found that TM practitioners had a 48% risk reduction in the composite of mortality, myocardial infarction, and stroke during 5.4 years of follow-up9.

Comparative Cost-Effectiveness

TM research demonstrates healthcare cost reductions ranging from 28% to 87% across multiple studies, significantly exceeding the 3RP program’s 43% reduction. The key advantages of TM include:

  • Longer study durations: TM studies span 5-14 years versus 1-year follow-up for other programs

  • Larger effect sizes: TM shows 28-87% reductions in various healthcare utilization categories

  • Broader population studies: TM research includes general population, high-cost patients, and elderly cohorts

  • Cardiovascular focus: TM has unique American Heart Association recognition for cardiovascular benefits 9, 10,12

The mounting evidence positions TM as perhaps the most cost-effective intervention for healthcare utilization reduction ever documented, with particular relevance for older adults seeking to maintain health while minimizing medical expenses.

The Future of Healthcare and Mind-body Therapies

Unfortunately, with few exceptions, the American health insurance industry lags behind much of the modern world in their willingness to pay for complementary forms of care. This in spite of the fact that a 2012 National Health Statistics Report that showed that well over 10 percent of Americans regularly engage in mind-body practices like yoga, meditation, tai chi and qigong within the past year to decrease stress and increase health and well-being.

Data from medical utilization studies offer resounding evidence that health insurance companies, systems and providers would better serve patients’ health and financial well-being by including mind-body therapies as standard forms of care. As the authors of the PLOS study state, “The focus in healthcare is shifting from high utilization of specialized care for the treatment of late-stage disease to an emphasis on patient-centered approaches and coordinated care teams that promote wellness, support self-care, provide preventive care and effective disease management … Our results indicate that mind-body interventions, such as 3RP, can reduce individual disease burden as well as the utilization of healthcare resources and are well suited to the changing healthcare environment.”

This sounds like a win-win scenario for both patients and the systems that serve them!
